A form of irritant contact dermatitis, Diaper Dermatitis, affects the area of skin which comes at contact of napkin or diaper. Cloth nappies are more likely to cause it. Though, it affects babies who wear napkins, but occasionally may affect adults and children who suffer from urinary or faecal incontinence & use diapers.
It is also known as Napkin Dermatitis/ Nappy rash/ Diaper rash/ Irritant Diaper Dermatitis/ Generic rash. Diaper dermatitis can be diaper associated or non diaper associated. Skin of babies, mostly of age group 9-12 months, which comes under the contact of diaper, gets easily affected because it is very tender and soft. Skin is the largest organ of human body. Skin is first line of defence system of human body. It forms a barrier and protects internal organs from external foreign bodies including bacteria, viruses, fungus.
The outermost layer of skin is called as stratum corneum. This layer contains dead cells called as Keratonicytes which are interlaced with lipids. This layer forms a barrier which is protective in nature. This layer protects skin from injury, water loss, and penetration of skin by infectious organisms.
In Diaper Dermatitis, this layer gets damaged. This layer is very thin in babies and hence more prone to be damaged easily. In Diaper Dermatitis, following things occur-
- Irritation to skin (by urine/ feces/ chemicals) causes over hydration & softening of skin
- Breakdown of outermost layer of skin (stratum corneum) due to skin irritation and frictional damage of skin.
- Damage to keratinocytes which are interlaced with lipids (secreted by stratum granulosum)
- This causes damage to Waterproof layer of skin (waterproof barrier)
Diaper Dermatitis affects babies mostly of 3 months to 1.5 years of age. Wet diaper easily softens and damages the outer protective layer of skin, which is damaged by acidic pH of urine. Ammonia in urine & stool cause further irritation and skin reacts in response, causing inflammation; resulting in diaper dermatitis.

Cause of Diaper Dermatitis is damage to protective barrier of skin.
- Irritants: Skin of babies which comes under the contact of diaper gets affected. Wet napkin irritates the soft skin of babies and affects the skin folds. Prolong wetness and change of skin pH due to acidic nature of urine and ammonia in faeces; may break the outermost skin layer. This causes damage to skin.
- Chafing: Chafing or rubbing of delicate skin of baby may cause rash. Ill or tight fitted diapers may rub against the skin.
- Retained soaps: Retained soap on reusable diapers due to improper washing may cause Diaper Dermatitis.
- Diarrhea: Frequent loose stools can irritate the skin and may cause Diaper Dermatitis.
- Infections: Skin infections like Candida albicans (caused by yeast or fungus), bacterial infections (impetigo), Seborrhoeic Dermatitis, are likely to cause dermatitis in diaper area.
- Allergens: Though rare, allergic reactions can be a cause behind Diaper Dermatitis. These allergic reactions can be from chemicals, perfumes, in soaps, diaper material, lotions.
- Nutritional deficiency: Very rarely, nutritional deficiency like biotin or zinc deficiency may cause dermatitis.
Diaper Dermatitis may trouble both you & your little one. If not treated on time, it can put the health of your child at risk.
- Pain: Diaper dermatitis can cause itching, irritation and pain. It can make them irritable and baby may cry frequently.
- Infection: Pathogens may enter the skin surface easily and cause infection which may spread to surrounding structures. Warm and moist buttock surface provides perfect breeding ground for bacteria and other organisms.
- Necrotizing fasciitis: Rarely, from the skin break, severely dangerous organism called GABHS (group A beta haemolytic streptococci) may penetrate inside the body and cause serious infection called Necrotizing fasciitis. It may require surgical debridement.
